Monday to Friday 9am till 5pm – with on call duties on a rota basis

This is an exciting opportunity to join as a Senior, in  Abacare as Care Coordinator in Swansea Area

You will be helping people to:

Remain in their own home and maximise their independence
Avoid going into hospital unnecessarily
Become independent as soon as possible after a stay in hospital

Coordinating the team of Support Workers as they provide support to clients in their own homes.
Use an electronic rostering system to allocate visits to Support Workers,
taking into account client needs and preferences whilst efficiently utilising the staffing resources available.
An eye for detail, strong IT skills and good problem-solving skills are vital for this position.
An element of delivering Personal Care is required with this role when on call.
